Every freelancer has had the client who says 'just a quick job' and then adds three revisions, two extra deliverables, and a deadline that moved twice. A client intake form stops that pattern before it starts. It is a short form you send to every new client before you quote, and it collects the details that decide the price, the timeline, and the scope — in writing, before any work begins.

A good intake form covers five things. First, the client's contact details and business — who you are working for and how to reach them. Second, the project itself: what they need, what success looks like, and what is explicitly not included. Third, the timeline and deadline, including any hard dates. Fourth, the budget range, which saves both sides from wasting time on a project that does not fit. Fifth, the practical details: brand assets, logins, file formats, and who approves the work.

Free intake form templates are available in Google Forms, Typeform, Notion, and plain Word or Google Docs formats. Google Forms is the easiest starting point: it is free, it saves responses in a spreadsheet automatically, and clients can fill it in without an account. Notion works well if you already manage clients there. Whatever format you choose, keep the form short — ten to fifteen questions is enough. A form that takes twenty minutes to complete will be ignored, and an ignored form is worse than no form at all.

The way you introduce the form matters as much as the form itself. Send it with a short, friendly message explaining that it helps you quote accurately and start faster — not as a bureaucratic hurdle. Most clients appreciate it, because it shows you are organised. If a client refuses to fill it in, that is useful information too: it usually means the project is vague, the budget is unrealistic, or the client is not ready to commit.

Use the intake form as the foundation for everything that follows. The answers become your proposal, your contract scope, and your project brief — so nothing gets lost between the first conversation and the final delivery. Review your form every few months and add the questions you keep asking clients by email. The form should evolve with your business, and after a year it will be one of the most valuable free tools you own.

An intake form does more than collect details — it reveals whether a client is worth working with. Vague answers are the first warning sign: a client who cannot describe what they need, or who writes "you decide" for every question, will be vague at every stage of the project. A budget that is far below your rate, combined with a demand for urgent delivery, is another red flag — it usually means the client has already been turned down by others.

Other warning signs include a client who will not name a decision maker, a deadline that is impossible without cutting corners, and a request for free sample work before any agreement. None of these mean you must refuse the job, but they do mean you should proceed carefully: quote a fair price, set clear terms, and ask for a deposit before starting. The intake form gives you this information before you have invested any time — which is exactly when it is most useful.

Build your intake form this week: pick a free template, adapt the questions to your services, and send it to your next new client. The first time it saves you from a vague brief or a surprise revision, you will wonder how you ever worked without one.
